Wrong and Hard to Correct


The half life of a mistake is surprisingly long. Obviously, it is a function of the importance of the mistake. But in a world of immutable ledgers, mistakes will live on forever somewhere on that ledger. 

Humans make all kinds of mistakes.

Catching mistakes quickly can sometimes reduce harm, but sometimes you only get one chance. Time doesn’t matter.

It is better to figure out how to prevent, preventable mistakes.
